The RACI model assigns the roles Responsible, Accountable, Consulted, and Informed to each task or decision.
RACI grew from responsibility assignment matrices in project management; no single authorship of the now-common acronym is reliably established. Teams use the matrix across projects, processes, and line organizations.
Put work packages or decisions in rows and participating roles in columns. Give each row at least one executing role and exactly one accountable role; make consultation and information deliberate. Then review handoffs, overload, and missing decision authority with participants.
Responsible is the role performing or coordinating the work.
Accountable owns final answerability and approves the result.
Consulted provides active input; Informed receives relevant information without a decision role.
RACI makes ownership and communication needs at interfaces visible. It supports handoffs and escalation but cannot replace real authority or collaboration; too many entries create bureaucracy.
